More Compounded Products Are Recalled Over Sterility Concerns Comments Off

The Food and Drug Administration has reported that Nora Apothecary & Alternative Therapies issued a voluntary recall regarding all units of certain compounded products produced by them that could be contaminated due to a lack of sterility assurance. According to many sources, multiple compounding pharmacies are recalling solutions over sterility concerns.

Since it’s uncertain whether the products re really contaminated or not, the company decided to issue the recall for all units of potentially hazardous compounded product dispensed either on April 19, 2013 or before. Healthcare facilities and patients that have received the products should contact the company at once to arrange the returning of the items. If you have been treated with compounding products that have been recalled and you have any health concerns, contact your healthcare provider as soon as possible.

The recall affects about 400 prescriptions compounded by the company and other 95 dosage units of several compounded products supplied to clinics or medical offices.

The company hasn’t received any incident reports or complaints related to illnesses or other adverse reactions related to its compounded products, however, patients are urged to contact the FDA if they have any illnesses to report.

Ford recalls 197,000 vehicles due to injury hazard Comments Off

Nearly 197,000 model year 2004-2007 Freestar and Mercury Monterey vehicles manufactured between March of 2003, and November of 2006 have been voluntarily recalled by Ford Motor Company due to an increased personal injury risk. According to the official recall notice issued by the National Highway Traffic Safety Administration, the seat anchor mechanism mounted to the rear wheel wells, potentially preventing the third row stowable seats installed in these vehicles from fully latching. In the even of an accident, an unlatched seat may come detached, increasing the risk of personal injury.

First Johnson & Johnson all-metal hip implant lawsuit goes to trial Comments Off

The first of more than 10,000 lawsuits filed against Johnson & Johnson in relation to complications from the all-metal ASR hip implants manufactured by J&J subsidiary DePuy went to trial in Los Angeles, California. According to the lawsuits, employees of DePuy were aware of possible safety issues stemming from the metal-on-metal hip replacements as early as 2004 when they originally began selling the product. Due to their abnormally high failure rate and the possibility that they may shed metal debris into the blood and tissue of patients resulting in medical complications, DePuy’s ASR all-metal artificial hip joints were recalled in 2010. While some legal experts predict that the company will eventually opt to settle the case globally, they are expected to fight a few of these cases in court.

According to the plaintiff’s attorney the patient’s all-metal hip implant caused elevated levels of cobalt and chromium in the patient’s blood, according to the plaintiff’s doctors, but attorneys for Johnson & Johnson argue that these blood levels were not high enough to cause medical complications.

Recently, the U.S. Food and Drug Administration issued a statement asking the manufacturers of all-metal hip implants to provide additional proof that these replacement joints are safe to patients before they continue to market them to medical professionals.

Explosion, fire hazard prompts Keystone RV recall Comments Off

In cooperation with a request from the National Highway Traffic Safety Administration, Keystone RV Company has issued a voluntary recall for at least 191 model year 2012 and 2013 Springdale travel trailers manufactured from October 18, 2011, through December 5, 2012 due to a potential suspension issue that may pose a fire hazard and an associated injury risk to vehicle occupants and others nearby. The suspension components installed in this vehicle may come into contact with the copper propane line connected to the vehicle’s cooking range, possibly causing a propane leak over time. If propane comes into contact with an ignition source, it may ignite, causing a potential fire or explosion hazard.

SUV crash causes concrete and metal to fall on passing vehicles in LA Comments Off

According to accident reports, a black Cadillac Escalade struck a guardrail on the 3rd Street overpass on the 110 Freeway Sunday evening, sending pieces of concrete and metal tumbling onto traffic passing below. The falling roadway debris was involved in at least six other traffic accidents, according to law enforcement authorities, but no injuries have been reported as of yet. Police were still looking for the occupants of the black SUV, who allegedly drove away from the scene of the accident, the following morning.

Fuel leak hazard prompts snowmobile recall Comments Off

Due to a fire hazard resulting from a potential fuel tank leak, Arctic Cat Inc. of Thief River Falls, Minnesota, has issued a voluntary recall for approximately 3,900 model year 2013 wide-track Bearcat series snowmobiles. To date, the company has received at least three reports of these snowmobiles leaking fuel at dealerships due to damage sustained during the production process. These products, made in the United States, were sold by authorized Arctic Cat dealers throughout the U.S. for an estimated price between $9,000 and $13,500.

Range Rover recalls 70 vehicles due to potential brake failure Comments Off

Approximately 70 model year 2012 LR2 and Range Rover Evoque vehicles manufactured on June 11, 2012 have been voluntarily recalled by Jaguar Land Rover North America, LLC due to the possibility that the rear brake caliper retaining bolts may be insufficiently torqued, possibly leading to the brake caliper detaching and preventing the vehicle’s brakes from functioning. Traffic accident rate spikes in Los Angeles during wet weather Comments Off

According to California Highway Patrol accident reports, light rain in Los Angeles has caused the traffic collision rate on area freeways to increase by nearly 400 percent when compared to a dry day. At least 188 accidents occurred on LA freeways between 12 am and 8:30am Thursday, the CHP reports, including a big rig that jack knifed on the Golden State Freeway and a collision involving multiple vehicles on the Ventura Freeway that killed one motorist and injured two others. The National Weather Service predicts that the rain will persist through the weekend, and law enforcement officers are urging motorists to drive cautiously.

Kirkland saute pans recalled due to possible laceration hazard Comments Off

In response to a request from the Consumer Product Safety Commission, Meyer Marketing Company has issued a voluntary recall for approximately 12,900 Kirkland Signature Six-Quart Saute Pans with glass lids. To date, the distributor has received at least 97 consumer complaints reporting that the glass lids on these pans have broken or shattered, posing a possible laceration hazard and injury risk. The pans included in this recall a black, six-quart saute pans with steel-rimmed tempered glass lids. They were sold at Costco Wholesale stores for approximately $35.

Auto safety regulators announce 2013 vehicle safety ratings Comments Off

The U.S. National Highway Traffic Safety Administration has announced crash test safety ratings for several model year 2013 vehicles, and while nine models of vehicles scored top ratings in all categories — some in a marked improvement over previously released designs — several other vehicles fell short of receiving top safety rating honors. The NHTSA’s crash testing procedure, conducted following the guidelines established in the  New Car Assessment Program,  entails assessing a vehicle’s safety based on its performance in a 38.5-mph side-impact crash test, 35-mph frontal crash test, a 20-mph side-impact test that simulates sliding into a large obstacle such as a utility pole, and a rollover evaluation primarily based on its center of gravity.

Ford Explorer and Hyundai Santa Fe Sport crossover sport utility vehicles, and Nissan Altima passenger sedans, some of the best selling vehicle models in recent years, received five-star safety ratings — the NHTSA’s top safety designation — along with the model year 2013 Cadillac XTS, Audi A4, Toyota Venza, Acura ILX, Infiniti M and Ford Taurus. The Hyundai Santa Fe Sport, which received a three-star safety rating for its model year 2012 design, was the most improved this year, but the majority of the vehicle models receiving top safety designations this year received only four-star ratings for their previous year’s designs.

The lowest scores received this year went to the extended cab versions of the Chevrolet Silverado  and GMC Sierra 2500 pickup trucks, which each earned two-stars in crash testing. The Coda EV electric car, which has not yet been fully tested, received a two star rating based on partial testing. NHTSA testing procedures became stricter in 2011, and automakers are continuing to adapt to receive higher scores.

As a personal injury attorney handling many lawsuits stemming from automotive accidents, I am hopeful that the auto industry’s safety standards continue to increase in coming years in order to reduce the number of tragic collisions that occur on American roadways every year.
